Accident Reporting

Purpose

To ensure all injuries, incidents, and near-misses are properly recorded and reviewed.

Scope

Covers all club activities involving players, coaches, volunteers, and visitors.

Responsibilities

  • Team Managers: Report all accidents to the Safety Officer.
  • Safety Officer: Review and monitor accident reports.

Procedures

  1. Complete an accident report form immediately after any incident.
  2. Inform the Safety Officer.
  3. Retain all accident forms securely.

Records

  • Accident reports logged in the club’s system.

Review

  • Accidents reviewed quarterly to identify trends or actions needed.
